Adnexal torsion: a single-center retrospective study of diagnosis and treatment

Aims: This study was conducted to review the clinical, laboratory, and ultrasound findings of patients with a confirmed post-operative adnexal torsion diagnosis in tertiary university hospital reflect management torsion.
 Methods: 44 (ages [15,44] median 30 yr) who had an operation at Ankara University Hospital Gynecology Obstetrics Clinics between October 2013 February 2021 whom preoperative intraoperatively were included this retrospective study. Patients’ complaints, existing risk factors, physical examination, radiological evaluated.
 Results: 26 (65%) complained about acute abdominal pain isolated on one side. Torsion observed right side 29 (66%) patients. Risk factors: History ovarian 1 patient (2%), vitro fertilization treatment 5 (11%) patients, 4 (9%) which also hyperstimulation syndrome 2 (5%) them polycystic ovary syndrome, finally, 28 increased size ultrasound. Ovarian blood flow not 25 (81%) 31 underwent transvaginal Doppler Preoperative examination showed anemia (Hb10000/mm³) (59%), increase neutrophil-lymphocyte ratio (NLR) (>3) 36 (82%). Laparoscopy performed 37 (84%) laparotomy 7 (16%).
 Conclusion: There is no objective diagnostic tool that can definitively lead torsion. It may be recommended use ultrasound, Doppler, NLR addition clinical anamnesis. Considering consequences delay treatment, most appropriate approach would perform laparoscopy if suspected. Additionally, patient’s age, menopausal status, pathology, desire for fertility are factors should considered decision, protection primary goal.
